New Patient Registration Forms
First, we’ll start with some paperwork. If you’re a new patient, please print and complete the New Patient Registration Form, the Adult Health History Form (or the Pediatric Health History Form), and the Notice of Privacy Practices before your first appointment. Doing so will save time when you arrive and enable us to attend to you sooner.
If you’re unable to print the forms before arriving, you’re welcome to fill them out at check-in. Please arrive at least 10 minutes early for your first appointment if you need to complete the paperwork.
