It’s time to feel confident and comfortable about healthcare once again. As a new patient, we connect you to the necessary medical, dental, behavioral, vision, and pharmacy services all within our clinic. Additionally, we’ll guide you to community services to support your wellbeing beyond our walls.
Please bring the following to your first appointment:
- Completed new patient registration forms
- Photo identification, which may include:
- Driver’s license or ID card issued by Kansas or another state
- U.S. Passport
- U.S. Military ID
- Employee badge or ID issued by a government office
- Student ID card from an accredited post-secondary education institution in Kansas
- Insurance card (refer to our list of accepted insurance plans)
- If you are an Native American or Alaska Native, please bring your tribal ID
- Proof of income, which may include:
- Most recent paycheck stub
- Unemployment benefits
- Food stamps benefits
- SSI benefits
- Previous year’s tax return
If you are unable to print the forms before arriving, you’re welcome to fill them out at check-in. Please arrive at least 10 minutes early for your first appointment if you need to complete paperwork.
